Key Responsibilities\n

• Conduct thorough assessments to evaluate and address the immediate and ongoing needs of patients.

• Engage in effective communication with supervisors, co-workers, families, and other stakeholders to keep everyone informed about clients' progress and any necessary adjustments to care plans.

• Develop and provide a structured schedule for patient appointments, including home visits and observations aimed at monitoring progress.

• Coordinate essential home visits, which may include chart reviews, staff interviews, and data analysis to ensure comprehensive understanding and planning.

• Ensure adherence to and effective implementation of treatment programs, adjusting strategies as needed based on patient feedback and outcomes.

• Participate actively in interdisciplinary team meetings, contributing insights that enhance overall patient care.

• Document monthly and quarterly progress notes to track patient progress and inform treatment modifications.

• Conduct training sessions to certify staff in program procedures and documentation practices, enhancing overall team competency.

• Carry out follow-up interviews and additional observations to continually assess and support individual patient needs.

• Maintain detailed records to ensure transparency and accessibility of information for all parties involved.

Patient Population / Client Focus\n

In this position, you will serve a varied clientele, including adults facing challenges with anxiety, depression, substance misuse, trauma, and family dynamics. You will be empowered to utilize your expertise in family therapy, trauma-informed care, or other specialized therapeutic areas to address the complex needs presented by our clients.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, Social Work, or a related field is required.

• An active and unrestricted Professional Clinical Counselor (LPCC) license in the state of California.

• A minimum of two years of relevant counseling experience, ideally in a clinical context, is required.

• Specialization in family therapy, couples therapy, substance misuse, or trauma-informed practices is highly preferred.

• Demonstrated knowledge of legal and ethical standards pertinent to mental health practice.
