Work Environment

This role will be based onsite within patients' homes throughout Cape Coral, providing a unique and fulfilling work setting. Key Responsibilities

• Conduct comprehensive evaluations of patients to determine their needs and treatment goals in a home care context.
• Develop and implement customized treatment plans using techniques such as therapeutic exercises, manual therapy, and modalities tailored to alleviate pain and enhance mobility.
• Maintain accurate documentation of treatment progress and outcomes, ensuring ongoing evaluation and adjustment of therapeutic goals as required.
• Educate patients and their families on proper use of assistive devices, self-care techniques, and overall strategies for functional improvement.
• Collaborate with caregivers, other healthcare professionals, and family members to ensure a holistic approach to patient care.
• Stay current in the latest physical therapy practices and advancements by attending workshops and participating in professional organizations.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will work with a diverse patient population, including those recovering from surgery, managing chronic conditions, or rehabilitating from injuries. Your expertise will play a critical role in improving their quality of life and functional abilities.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Must hold a state license in Physical Therapy.
• Current CPR certification is required.
• Excellent verbal and written communication skills to effectively interact with patients, families, and interdisciplinary teams.
• Proficiency in using computer software for documentation and communication.
• Ability to adapt and remain flexible in a variety of home care settings to meet patient needs.

Preferred Experience

• Two years of experience as a licensed Physical Therapist is preferred.
• While experience is preferred, we also welcome applications from new graduates eager to expand their knowledge and skills through our mentorship programs.

Compensation and Benefits
