Work Environment

This role offers an onsite work environment where you will engage directly with patients in their homes, providing personalized care in a variety of settings. Scheduled Monday to Friday, you can expect a blend of hands-on patient care with opportunities to develop and execute innovative rehabilitation programs.

Key Responsibilities

• Conduct comprehensive evaluations to determine patients' physical capabilities and limitations, including reviewing medical histories and performing standardized assessments.
• Develop individualized treatment plans in collaboration with patients and families, ensuring clear communication regarding findings and treatment strategies.
• Implement evidence-based treatment protocols that cater to the unique needs of each patient, continually assessing their progress and adapting techniques as necessary.
• Document patient care activities meticulously, including evaluation results, intervention strategies, progress notes, and discharge planning to maintain a clear and thorough patient record.
• Collaborate effectively with multidisciplinary healthcare professionals to coordinate care and address any changes in patient conditions that necessitate revised treatment plans.
• Engage in community outreach and marketing initiatives to promote our rehab services and improve access to care, while also participating in departmental meetings and contributing to strategic planning.
• Serve as a mentor for students and junior staff, guiding them through clinical practices and promoting a culture of continuous learning.

Patient Population / Client Focus

In this role, you will primarily work with patients requiring orthopedic rehabilitation—this includes individuals recovering from surgeries, sports injuries, stroke, and other musculoskeletal conditions. You will tailor your treatment plans based on each patient's specific health history, lifestyle needs, and personal rehabilitation goals, ensuring a patient-centered approach that fosters engagement and motivation.

Qualifications

• Bachelor's degree in Physical Therapy; a Doctorate in Physical Therapy is preferred.
• A valid state license to practice Physical Therapy is required for the state of Florida.
• Current certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• A passion for providing compassionate care and improving patient outcomes in a home care setting.

Preferred Experience

While prior experience is not mandatory, we welcome applications from candidates with clinical exposure in orthopedic settings. Experience using electronic medical record (EMR) systems and familiarity with evidence-based practice guidelines will be considered a plus.

Compensation and Benefits
