Work Environment\n

This position operates in a remote setting that allows for flexibility while maintaining a structured full-time schedule. Our work hours are typically Monday to Friday from 8:00 AM to 4:30 PM. Occasionally, evening and weekend hours may be required to address client needs, which emphasizes our commitment to providing responsive and accessible care. You will engage with clients and their families through virtual platforms, utilizing electronic health record (EHR) systems and telehealth methodologies, ensuring uninterrupted access to valuable support services.

Key Responsibilities\n

• Conduct in-depth assessments of clients' psychosocial needs, employing empathetic listening and observation skills to evaluate emotional, mental, and social influences impacting their well-being.

• Develop and implement personalized care plans in collaboration with clients and their families, addressing specific challenges such as anxiety, depression, and life transitions.

• Provide ongoing counseling services, emotional support, and resources, empowering families to navigate the complexities of life changes and challenges effectively.

• Advocate for clients’ rights and access to essential community resources, ensuring comprehensive support for their mental health and overall well-being.

• Coordinate efforts with interdisciplinary teams, including educators, healthcare providers, and family members, to connect clients with integrated services that enhance their care.

• Monitor client progress through regular check-ins, assessing living conditions and ensuring safety, particularly for vulnerable populations.

• Educate clients and their families about local support groups and community programs, fostering increased social engagement and connection.

• Document all client interactions meticulously, maintaining up-to-date records of assessments, treatment plans, and progress notes to ensure high-quality care.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
