Key Responsibilities\n

• Deliver intensive case management services tailored to a small caseload of children and families affected by terminal illness.

• Facilitate patient scheduling for medical, behavioral health, and specialist appointments, helping to coordinate comprehensive care plans.

• Provide administrative support such as reviewing patient records, screening eligibility for services, and tracking progress toward treatment goals.

• Conduct home and community visits as necessary to assess patient needs and provide direct support.

• Work closely with the interdisciplinary care team comprising behavioral health providers, medical professionals, and community partners to ensure seamless care coordination.

• Document all encounters and services provided in the electronic health record system and maintain accurate records.

• Engage in interdisciplinary meetings for case conferencing and administrative purposes, contributing insights and sharing best practices.

Qualifications\n

• Active LCSW license in the state of Pennsylvania is required.

Compensation and Benefits\n

We offer a competitive salary ranging from $80,000 to $90,000 annually, commensurate with experience.
