Work Environment\n

This position is set in a fully remote work setting, allowing you the flexibility to manage your schedule. While you will engage with patients and families predominantly through telehealth platforms, you may also have opportunities to provide in-person consultations at our Hospice Center in Fresno, California. This role requires strong technological proficiency to effectively utilize various remote communication tools and electronic health record systems.

Key Responsibilities\n

• Conduct thorough assessments of clients, including psychological evaluations and mental health screenings, to identify their unique needs and develop tailored treatment plans.

• Implement evidence-based therapeutic interventions aimed at mental health recovery, utilizing modalities such as CBT, DBT, or mindfulness practices appropriate for the client.

• Document all clinical interactions meticulously in compliance with legal and ethical standards, ensuring accurate records of diagnoses, treatment plans, and progress notes.

• Work collaboratively with a diverse multidisciplinary team, including medical professionals and fellow social workers, to design and implement holistic care strategies.

• Educate clients and their families about coping strategies, mental health resources, and support networks, helping them to navigate the complexities of mental health management.

• Engage in continuous professional development, staying updated on emerging mental health practices and integrating relevant innovations into treatment approaches.

Patient Population / Client Focus\n

The LCSW will primarily serve clients dealing with a range of mental health challenges, including but not limited to anxiety, depression, trauma, and grief. As part of our commitment to serving a vulnerable population, you will work closely with individuals receiving hospice care, aiming to support their emotional and psychological needs during a critical time in their lives.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, reflecting a solid foundation in clinical practices.

• Current LCSW license in good standing, demonstrating your qualifications and commitment to the profession.

• Minimum of 2 years of direct experience in clinical social work, with a proven track record in assessment, treatment planning, and delivering effective mental health interventions.

• Strong interpersonal, communication, and active listening skills to build rapport and trust with clients and their families.

• Proficient in using digital tools and remote technology platforms, enhancing your ability to deliver care from a distance.

Preferred Experience\n

Experience working in a hospice setting or with patients facing end-of-life issues is highly desirable. Familiarity with trauma-informed care practices will further enhance your ability to meet the needs of our clients effectively.

Compensation and Benefits\n

This position offers a competitive salary range of USD 90,000 – 100,000 annually, commensurate with experience.
