Key Responsibilities\n

• Conduct thorough assessments and create individualized treatment plans that include pre-crisis and crisis intervention strategies, behavioral guidelines, and medication treatment plans.

• Maintain timely and accurate documentation in compliance with regulatory standards and practice protocols.

• Utilize evidence-based screening tools to ensure effective treatment modalities are employed in accordance with clinical guidelines.

• Collaborate with cross-functional teams to design and implement positive behavior support plans tailored to the needs of geriatric patients.

• Actively participate in licensing and accreditation processes, ensuring we meet and exceed the established criteria.

• Serve as a key liaison between clients and relevant service systems, state agencies, and community partners, facilitating a smooth continuum of care.

Qualifications\n
