Key Responsibilities\n

• Conduct comprehensive assessments to evaluate clients' mental health needs and create individualized treatment plans.

• Maintain open channels of communication with supervisors, direct care staff, family members, and significant others to facilitate client progress updates.

• Organize and document a schedule of appointments, home visits, and therapeutic observations to monitor client advancement and adherence to treatment.

• Perform in-home assessments when necessary, including chart reviews and interviews with staff members, to ensure a holistic understanding of each client’s situation.

• Evaluate the effectiveness of implemented program plans and make necessary adjustments to address clients' evolving needs.

• Actively participate in interdisciplinary team meetings to discuss case strategies and share insights regarding client care.

• Deliver timely progress notes summarizing clients' statuses on a monthly and quarterly basis, ensuring compliance with organizational policies.

• Facilitate training sessions for staff members to ensure they are well-versed in implementing program protocols and proper documentation procedures.

• Support clients with follow-up interactions and additional home visits as needed to provide ongoing care and assessment.

• Serve as a liaison between clients and the broader mental health community, helping to connect individuals with additional resources as necessary.

Qualifications\n
