Key Responsibilities\n

• Engage directly with clients suffering from major mental illness or severe emotional disorders, providing both in-office and community-based support.

• Conduct comprehensive assessments using evidence-based screening tools and develop tailored care plans that facilitate access to mental health services and resources.

• Utilize a trauma-informed approach and best practices to deliver focused prevention, treatment, and recovery services.

• Participate actively as a member of a multidisciplinary team, contributing to collaborative goals and respecting diverse perspectives and backgrounds.

• Provide crisis intervention services as necessary, supporting clients through acute difficulties and ensuring their immediate safety and well-being.

• Maintain accurate and timely documentation, including case notes, treatment plans, and discharge summaries, to comply with organizational and legal standards.

• Monitor and track client progress while staying engaged with families and other stakeholders to ensure continuity of care.

Qualifications\n
