Key Responsibilities

As a Physical Therapist at Valera, your daily responsibilities will include:

• Conducting comprehensive evaluations of patients' physical capabilities through diagnostic assessments and functional tests, allowing you to develop tailored treatment plans.
• Administering manual therapeutic exercises and guiding clients through non-manual interventions, motivating them to achieve their rehabilitation goals.
• Overseeing the work of Physical Therapy Assistants, ensuring their adherence to the established plan of care and facilitating effective communication with them and other healthcare team members.
• Evaluating home environments to recommend modifications that support clients' independence and their rehabilitation journeys.
• Preparing and maintaining thorough clinical records, including initial evaluations, progress notes, and discharge summaries, complying with OASIS requirements where applicable.
• Collaborating with an interdisciplinary team in case conferences to assess and update the total treatment plan for each client, ensuring a holistic approach to care.
• Providing education to clients and their families on physical therapy practices and device usage to empower them in their recovery.
• Regularly observing and documenting changes in patients' conditions, providing critical feedback to physicians and case managers for potential adjustments to the treatment plans.

Patient Population / Client Focus

Your work will primarily focus on clients recovering from various physical challenges, whether due to surgery, injury, or chronic conditions. By delivering personalized therapy, you will assist individuals in regaining functionality and independence, making a significant impact on their overall health and well-being.

Qualifications

To be considered for this role, candidates must meet the following qualifications:

• A Doctorate degree in Physical Therapy (DPT) from an accredited institution.
• A valid physical therapy license in the State of New Jersey.
• Current CPR certification to ensure patient safety in emergency situations.
• A valid driver's license along with automobile insurance for travel to client homes when required.

Preferred Experience

Compensation and Benefits
