Key Responsibilities\n

• Conduct comprehensive assessments of clients’ social, emotional, and psychological needs, utilizing tools and methodologies tailored for hospice settings.

• Provide both individual and family counseling, employing evidence-based practices specific to grief and end-of-life issues.

• Develop and implement personalized service plans aimed at achieving clients’ health goals and enhancing their quality of life.

• Advocate tirelessly for clients by navigating healthcare systems, ensuring access to necessary community resources, and addressing social justice issues pertinent to underserved populations.

• Collaborate with a diverse array of stakeholders including healthcare professionals, volunteers, and community organizations to strengthen service delivery and outcomes.

• Engage in program development and evaluation, contributing to grant reporting and documentation that supports Mice Healthcare’s mission.

• Participate actively in training sessions and community outreach activities to raise awareness about hospice care and the services provided by Mice Healthcare.

Patient Population / Client Focus\n

In this role, you will work primarily with patients receiving hospice care, who are often facing terminal illnesses. You will provide vital emotional support to both patients and their families during one of the most difficult times of their lives, helping them cope with grief, loss, and the complexities surrounding palliative care. Your interventions will be pivotal in ensuring that patients receive the compassionate care they deserve, while also empowering families to make informed decisions and express their feelings in a supportive environment.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

• Strong understanding of social justice issues and trauma-informed care principles.

• Excellent communication skills, with an emphasis on active listening, empathy, and the ability to build rapport with clients and families.

• Demonstrated organizational skills and effectiveness in both independent and team-oriented settings.

Compensation and Benefits\n
