Key Responsibilities\n

• Conduct thorough psychosocial assessments, gathering critical information about clients’ mental health concerns, strengths, and available resources.

• Utilize evidence-based therapeutic modalities such as cognitive-behavioral therapy, solution-focused therapy, and trauma-informed care to deliver individual, family, and group therapy sessions.

• Collaborate actively with the hospice care team, including physicians and nurses, to develop and implement personalized treatment plans that address the unique needs of each patient.

• Facilitate engaging group therapy sessions and psychoeducation classes that focus on coping mechanisms and support systems for individuals and families dealing with grief and loss.

• Implement crisis interventions and provide thorough risk assessments, ensuring safety plans are in place for clients experiencing acute distress.

• Maintain precise documentation of client interactions and treatment progress in line with ethical and legal standards, using the latest healthcare software tools.

• Offer consultation and support to family members to enhance their understanding and ability to cope with the emotional challenges associated with hospice care.

Patient Population / Client Focus\n

The primary focus of this role will be on patients receiving hospice care, which includes individuals facing terminal illnesses and their families. You will be tasked with addressing complex emotional needs and providing guidance through the nuances of grief, loss, and end-of-life transitions. Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The salary range for this position is between USD 80,000 and 90,000 annually, commensurate with experience, and accompanied by sign-on bonuses.
