Key Responsibilities\n

As a Licensed Professional Counselor at Veterans Crisis Intervention, Inc., your key responsibilities will include:

• Collaborating with primary care providers to identify, assess, and manage mental health and psychosocial issues, acting as an integral member of the healthcare team.

• Conducting comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments to inform treatment planning.

• Delivering brief, focused interventions for patients in need of immediate mental health services and establishing therapeutic rapport quickly.

• Providing recommendations to primary care providers on treatment goals and course adjustments based on patient status.
\n
• Initiating follow-ups to track patient progress and adapt treatment approaches as necessary, including developing and monitoring relapse prevention plans.

• Identifying at-risk patients and formulating effective strategies to avoid deterioration in their mental health status.

• Coordinating patient care by linking individuals with appropriate resources within the healthcare system, whilst adhering to confidentiality guidelines.

• Documenting patient progress accurately in treatment charts and keeping primary care providers updated on developments in care.

• Fostering behavioral changes in patients by providing education and self-management techniques that promote healthier lifestyle choices.

• Assisting patients in adhering to medical treatment recommendations provided by primary care physicians, including strategies to cope with medication side effects.

• Adhering to the ethical codes and policies established for clinical professionals while carrying out additional duties as assigned by management.

Patient Population / Client Focus\n

Your role as an LPC will involve working primarily with veterans and individuals from the local Tucson community, addressing a wide range of mental health challenges. Qualifications\n

To be successful in this role, candidates must possess the following qualifications:

• A current and active LPC (Licensed Professional Counselor) license in Arizona.
