Work Environment

The work setting is onsite, providing care directly in patients' homes across Jacksonville, Florida. This unique environment requires flexibility and adaptability as you navigate different living conditions and patient needs. You will be part of a collaborative team that includes direct communication with multidisciplinary professionals to ensure cohesive patient care. The typical weekly schedule involves 40 hours of patient interactions, with variable weekend hours as needed.

Key Responsibilities

• Conduct comprehensive evaluations of patients with functional limitations and disabilities.
• Develop, implement, and modify individualized treatment plans based on assessment outcomes and patient goals.
• Document all evaluations, treatments, and progress notes accurately to ensure compliance with healthcare standards and facilitate communication among the healthcare team.
• Educate patients and their families on treatment protocols, expected outcomes, and self-management strategies to enhance recovery.
• Collaborate closely with an interdisciplinary treatment team, including physicians, nurses, and other healthcare providers to coordinate care effectively.
• Participate in community outreach programs and contribute to professional development sessions to foster a culture of continuous learning.
• Assist in supervising and mentoring support staff, including Physical Therapy Assistants and aides, to maintain high standards of practice.
• Engage in ongoing professional development through continuing education and required certifications.

Patient Population / Client Focus

This position focuses on a diverse patient population in home care settings, ranging from post-operative orthopedic cases to neurological health challenges. As a Physical Therapist, you will assess and treat individuals across different age groups, addressing unique needs that may include rehabilitation post-surgery, management of chronic conditions, and recovery from acute injuries. Qualifications

• A bachelor's degree in Physical Therapy from an accredited institution.
• A valid State License in Physical Therapy and current Basic Life Support (BLS) certification.

Compensation and Benefits

This rewarding position offers a competitive salary ranging from USD 100,000 to 120,000 per year, commensurate with experience and qualifications.
