Urban Movement Rehab & Wellness Center is seeking a dedicated and skilled Full-Time Orthopedic Physical Therapist to join our team in Jacksonville, Florida. Work Environment

This position entails providing care in a home care setting, offering you the unique opportunity to engage with patients in their comfort zones. The role translates to varied environments, and flexibility is key to meeting the diverse needs of our patients. We foster a collaborative team atmosphere, currently consisting of two physical therapists and a physical therapy assistant, ensuring a supportive and communicative workspace.

Key Responsibilities

• Evaluate patients' conditions, needs, and goals to create effective treatment plans tailored to individual progress.
• Implement therapeutic interventions that contribute toward enhancing mobility, strength, and overall wellness, such as therapeutic exercises, manual therapy techniques, and patient education.
• Manage a planned caseload, working with one patient per hour to provide focused and quality care.
• Document patient progress and treatment outcomes rigorously, maintaining accurate electronic medical records using WebPT EMR Software.
• Work closely with patients to develop and adjust personalized wellness programs that promote a healthy lifestyle, including strength training and flexibility exercises.
• Provide education on injury prevention and self-management techniques to empower patients in maintaining their health.
• Collaborate with other healthcare professionals, when necessary, to ensure comprehensive and coordinated care plans.
• Continuously assess and refine treatment plans based on patient feedback and therapy outcomes.

Patient Population / Client Focus

Our patient population primarily consists of individuals seeking rehabilitation from orthopedic injuries, surgeries, and conditions affecting mobility. You will work with a diverse demographic, ranging from active young adults to elderly individuals, each requiring personalized engagement and treatment strategies. Our focus on proven, evidence-based practices ensures that patients receive the most effective and appropriate care to achieve their recovery goals.

Qualifications

• A Doctorate in Physical Therapy (DPT) is required.
• A valid Florida Physical Therapist license is mandatory.
• A clean professional record, with no history of disciplinary actions against your license.
• Demonstrated expertise in orthopedic rehabilitation, particularly in outpatient settings.
• Strong skills in manual therapy, exercise prescription, and patient communication are essential.
• Familiarity with WebPT EMR Software is preferred, as efficient documentation is crucial in our practice.

Preferred Experience

• Previous experience working in home care or similar settings, providing a personalized approach to patient care.
• Proficiency in developing and executing individualized treatment plans based on comprehensive assessments.
• Experience collaborating within a multidisciplinary team, highlighting skills in teamwork and communication.
• Engagement in ongoing professional development and continuing education to stay current with best practices in physical therapy.

Compensation and Benefits

The annual salary for this position ranges from USD 100,000 to 120,000, commensurate with experience and qualifications.
