Work Environment

This position is onsite and entails providing therapy services directly within client homes. Key Responsibilities

As an Orthopedic Home Care Physical Therapist, your daily responsibilities will encompass:

• Directing physical therapy interventions focused on pain relief, disability prevention, and muscle function restoration.
• Overseeing Physical Therapy Assistants, ensuring proper execution of care plans.
• Conducting thorough evaluations of patient functionality, utilizing diagnostic tests to inform treatment.
• Assessing home environments to enhance rehabilitation efforts and recommending modifications as necessary.
• Administering both manual therapies and instructing clients in therapeutic activities.
• Tracking and reporting changes in patient conditions, while collaboratively adjusting care plans with physicians and case managers.
• Preparing clinical documentation, including evaluations, progress notes, and discharge summaries, that adheres to OASIS standards.
• Engaging with interdisciplinary teams to coordinate and enhance total patient care.
• Educating clients and families on the use of therapy devices and overall care plans.
• Completing functional assessments and adjusting care plans as required by each client's progression.

Patient Population / Client Focus

Your work will primarily focus on patients recovering from orthopedic conditions, such as joint replacement surgeries, fractures, and musculoskeletal injuries. By providing personalized therapy in home settings, you will directly contribute to their physical rehabilitation journeys, helping them regain independence in their daily activities.

Qualifications

The ideal candidate must possess:

• A Doctorate in Physical Therapy from an accredited institution.
• Current licensure in good standing to practice physical therapy in Florida.
• CPR certification and a valid driver's license with automobile insurance.
• Strong interpersonal communication skills for effective patient and family engagement.
• Proficiency in navigating federal and state healthcare regulations.
• A commitment to clinical excellence and lifelong learning.
• The ability to work collaboratively within an interdisciplinary team.

Preferred Experience

A minimum of one year of experience in physical therapy, preferably within acute care or home care settings, is highly desirable. Familiarity with home healthcare workflows and needs assessment will be considered an asset.

Compensation and Benefits

Compensation for this role ranges from $100,000 to $120,000 annually, commensurate with experience and qualifications.
