Key Responsibilities\n

• Provide direct counseling and educational support to patients and their families, helping them navigate both emotional and practical aspects of mental health challenges.

• Advocate for patients' rights and needs, ensuring they understand available protective services and resources.

• Document and monitor patient progress using electronic medical records, ensuring that care goals are communicated and met effectively.

• Participate in interdisciplinary team meetings to discuss patient care strategies and potential interventions for high-risk patients.

• Facilitate transition conferences with patients and families to plan appropriate care and ensure smooth transitions to various levels of support.

• Utilize assessment tools to identify patients at risk for readmission, developing proactive strategies to mitigate these risks.

• Assure compliance with legal and ethical obligations regarding reporting vulnerabilities or suspected cases of abuse.

Qualifications\n

• A Master's or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work (e.g., MA, MS, MSW, MFT, MHC, PhD, PsyD) from an accredited program.

• An active independent license in Mental Health Therapy (LCSW, LMFT, LPC) issued in the state of residence.

Compensation and Benefits\n
