This role will primarily be executed in a climate-controlled remote setting, but the LCSW will also interact closely with department members and healthcare providers within our Hospice Center. Daily responsibilities involve both individual patient sessions via telemedicine and occasional in-person meetings to facilitate dynamic care coordination. Key Responsibilities\n

The Licensed Clinical Social Worker will engage in the following primary responsibilities:

• Conduct thorough assessments to evaluate patients' mental health status using standardized tools, including the Diagnostic and Statistical Manual of Mental Disorders.

• Formulate individualized treatment plans that incorporate cultural diversity and the unique needs of our patient population.

• Provide evidence-based therapeutic interventions while continuously evaluating patient progress and adjusting treatment plans as needed.

• Collaborate with patients and their families to ensure a strong support system and shared decision-making in their care evolution.

• Maintain accurate treatment records and reports in compliance with regulatory standards and the organization's protocols.

• Foster collegial relationships with other clinicians and care providers to ensure optimal patient pathways and referrals.

• Educate and mentor interns or volunteers within the clinical setting, contributing to their professional development as they navigate the field of social work.

Qualifications\n
