Key Responsibilities\n

• Evaluate patients' psychosocial needs through in-depth assessments and develop individualized treatment plans tailored to their specific circumstances.

• Provide case management and short-term counseling, offering crisis intervention as necessary to support patients during pivotal moments in their mental health journeys.

• Assist patients in navigating complex healthcare systems, empowering them to complete forms for entitlements and supplemental insurance, ultimately reducing reliance on emergency assistance programs.

• Coordinate access to prescription cost assistance programs through pharmaceutical companies and community organizations, ensuring that finances do not hinder treatment adherence.

• Act as a steadfast patient advocate in interactions with community agencies and resources, fostering relationships that benefit patient care.

• Maintain diligent documentation, including accurate patient records, progress notes, and reports to track patient outcomes and inform ongoing care.

• Facilitate transportation assistance for patients when needed, demonstrating comprehensive care that extends beyond virtual interactions.

• Engage actively in team meetings and clinical case conferences to contribute insights, share updates on patient progress, and collaborate on best practices in mental health care.

Patient Population / Client Focus\n

You will work closely with a varied patient demographic, including individuals coping with anxiety, depression, trauma, and other mental health conditions. Qualifications\n

• A Master's degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Applicants must possess an active and independent license: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• A minimum of two (2) years of professional experience in the mental health field is essential.

Preferred Experience\n

Compensation and Benefits\n
