Key Responsibilities\n

As a Licensed Professional Counselor, you will undertake the following responsibilities:

• Collaborate with primary care providers to recognize, manage, and treat mental health and psychosocial issues as a vital member of the healthcare team.

• Conduct thorough patient intakes, focusing on diagnostic and functional evaluations, and collaborate with primary care providers to develop individualized treatment plans.

• Deliver brief, focused interventions for patients requiring immediate mental health services and provide follow-up assessments to track patient progress.
\n
• Advise primary care providers on mental health referrals for patients needing specialized care beyond the primary care setting.
\n
• Develop relapse prevention plans to promote stability and help patients achieve long-term wellness.
\n
• Identify at-risk patients and implement plans to mitigate the worsening of their mental health conditions.
\n
• Coordinate with additional mental health professionals and treatment providers, ensuring comprehensive care for patients.
\n
• Document patient progress and treatment plans in accordance with established clinical standards and regulatory requirements.

• Foster strong relationships with patients and their families to encourage adherence to treatment regimens and enhance overall patient outcomes.

• Participate in team meetings, providing insights on behavioral health trends, outcome evaluations, and program development.

Qualifications\n

Compensation and Benefits\n
