Key Responsibilities\n

• Conduct comprehensive assessments tailored to the specific needs of each patient, providing a solid foundation for their care plans.

• Maintain open and effective communication with supervisors, all team members, and associated caregivers to share updates on patient progress.

• Provide a structured schedule of appointments and home visits for thorough monitoring of progress, ensuring accountability in treatment.

• Coordinate necessary home visits to collect pertinent observational data, conduct chart reviews, and facilitate interviews that inform ongoing treatment plans.

• Ensure that established program plans are implemented effectively to meet patient needs.
\n
• Actively participate in Interdisciplinary Team Meetings and other necessary forums to enhance collaboration and care quality.
\n
• Regularly complete and document monthly and quarterly progress notes to maintain accurate and transparent records of patient interactions.
\n
• Organize training sessions for staff to ensure competency in program plan implementation and documentation protocols.
\n
• Conduct follow-up interviews and home visits, providing continuous support while monitoring individual needs.
\n
• Act as a liaisons within the public and mental health community to foster relationships and share organizational development objectives.

• Assist in developing and refining training materials aimed at improving service delivery and program effectiveness.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field.
