Key Responsibilities\n

• Deliver virtual and telephonic educational support to patients and their caregivers, ensuring clear communication and understanding of treatment plans.

• Act as the primary point of contact for patients and liaise with healthcare professionals to coordinate comprehensive care.
\n
• Assess and understand the unique support needs and communication preferences of patients to create a tailored, seamless experience for those in treatment.
\n
• Instruct patients on program policies and procedures; maintain accurate documentation in clinical records to ensure compliance and continuity of care.

• Regularly review and update patient progress notes, ensuring clinical charts reflect critical patient information and adherence to treatment protocols.

• Participate in multidisciplinary team meetings to discuss patient progress and collaboratively plan interventions and follow-ups.

Patient Population / Client Focus\n

You will work primarily with individuals undergoing detoxification, addressing both their psychological and emotional challenges associated with substance use disorders. Your role will be pivotal in guiding these patients through significant transitional phases, ensuring they receive the emotional support needed during recovery.

Qualifications\n

• A Master’s degree in Social Work, Counseling, or a related field is required.

• Active LCSW license valid in California.

• Minimum of three years of experience in a clinical counseling setting with a focus on mental health.
