Key Responsibilities\n

• Conduct thorough ongoing assessments to fully understand and meet the needs of patients through personalized treatment plans.

• Maintain consistent communication with supervisors and direct care staff about patient progress, ensuring all team members are aligned on treatment goals.

• Develop and share a tentative schedule with supervisors detailing appointments, home visits, and observations to monitor patient progress closely.
\n
• Organize and coordinate home visits, ensuring all necessary data is collected and reviewed, including direct observations and staff interviews.
\n
• Ensure the effective implementation of program plans, providing guidance and adjustments as necessary to meet evolving patient needs.
\n
• Actively participate in Interdisciplinary Team Meetings to discuss patient progress and care strategies, facilitating a comprehensive treatment approach.
\n
• Document patient progress through monthly and quarterly notes, maintaining meticulous records accessible to authorized personnel.
\n
• Lead training sessions aimed at certifying team members in program implementation and documentation practices.
\n
• Conduct follow-up interviews and home visits to offer ongoing support and to ensure patients are progressing towards their personal goals.

• Cultivate relationships within the mental health community, acting as a liaison to enhance support and resource availability.

• Assist in developing procedures for effective training materials, continually striving for improvement in the delivery of services to patients.

Patient Population / Client Focus\n

You will primarily be providing services to a diverse population, including both adults and youth. Your work will encompass a variety of therapeutic approaches tailored to various mental health issues, including anxiety, depression, trauma, and family dynamics. Your expertise will be crucial in facilitating individual therapy sessions, as well as group therapy initiatives when applicable.

Qualifications\n

• A Master’s or Doctorate in Counseling, Psychology, or a related field is required.

• Current, active, and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona.

• A minimum of two years of counseling experience is essential, particularly in a clinical setting.

• Specialization in areas such as family therapy, couples therapy, substance misuse, or trauma is strongly preferred.

• Robust knowledge of legal and ethical standards in mental health practice.
