Key Responsibilities\n

The LPC will play a vital role in our primary care team, focusing on the mental health of patients through the following responsibilities:

• Conduct thorough patient intakes, including diagnostic and functional evaluations, to formulate appropriate treatment recommendations for the primary care provider.

• Provide brief, focused interventions tailored to patients requiring mental health services, ensuring adherence to established Federally Qualified Health Center (FQHC) guidelines.

• Monitor patient progress, initiate follow-ups to assess treatment efficacy, and adjust care plans as necessary.

• Identify at-risk patients, develop strategic prevention plans, and facilitate referrals to specialty mental health resources when required.

• Document all patient interactions and treatment plans in accordance with clinical record-keeping requirements, maintaining accurate logs of progress and treatment outcomes.

• Collaborate with primary care providers to create comprehensive treatment plans that address both physical and behavioral health needs.
\n
• Educate patients on self-management strategies to empower them in their recovery processes and promote healthier lifestyle changes.

• Provide consultation to clinic management and team members regarding behavioral services and evaluation outcomes.

• Uphold ethical standards of conduct and actively engage in ongoing professional development and compliance training.

Patient Population / Client Focus\n

Qualifications\n

To be considered for this position, candidates must possess the following qualifications:

• A minimum of a Master’s Degree in Clinical Counseling from an accredited institution.

• A current and valid LPC (Licensed Professional Counselor) license.

• Comprehensive knowledge of mental health assessment, treatment planning, and psychotherapeutic techniques, including familiarity with DSM-IV diagnoses.

• Strong familiarity with the impact of physical and mental health issues on individuals, families, and communities.
