Key Responsibilities\n

• Collaborate with primary care providers to identify, address, and manage mental health and psychosocial concerns, effectively functioning as a core member of the healthcare team.

• Conduct comprehensive patient intakes, focusing on diagnostic evaluations and functional assessments, to inform treatment recommendations aligned with best practices.

• Deliver brief, focused interventions for patients in need of mental health support, ensuring immediate assistance when necessary.
\n
• Consult with primary care doctors to determine the suitability of patients for continued care within the clinic versus referral to specialized mental health facilities.
\n
• Initiate follow-up communications to evaluate patients’ progress and adjust treatment plans based on their evolving needs.
\n
• Develop relapse prevention strategies and support patients in maintaining stable functioning in their daily lives.
\n
• Identify patients at risk of deteriorating mental health and create proactive care plans to mitigate these risks effectively.
\n
• Facilitate coordination of services among various healthcare providers, integrating behavioral health services into overall patient care.
\n
• Document patient progress and diagnostic information meticulously, ensuring accurate record-keeping that aids in developing tailored treatment plans.

• Encourage patients to embrace self-management skills and provide educational resources to empower them in their recovery journey.

• Advise clinic leadership and colleagues on behavioral health services and suggest areas for program evaluation and improvement.

• Support patients in adhering to medical treatments initiated by their primary caregivers, including offering coping strategies for medication management.

• Uphold the highest ethical standards as per the Code of Ethical Conduct for Clinical Professionals.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• Current Licensure as a Licensed Professional Counselor (LPC) in Arizona.
\n
• Thorough understanding of mental health assessment methods, treatment planning, and therapeutic modalities including but not limited to DSM-IV diagnoses and behavioral therapy.

• Ability to establish rapport and effective communication with patients, their families, and fellow team members.
\n
• Strong documentation skills for maintaining clinical records and preparing detailed reports.
\n
• Mental and physical capability to fulfill the demands of the role.
\n

Preferred Experience\n

• Bi-lingual proficiency in English and Spanish is preferred, as it enhances the ability to serve a broader patient base.

• Experience working in a clinical setting with diverse populations and familiarity with community resources.

Compensation and Benefits\n

This position offers a competitive salary ranging from USD 87,000 to 105,000 annually, reflecting your experience and the value you bring to our organization.
