Key Responsibilities\n

• Deliver clinical consultations to fellow clinicians regarding a range of mental health challenges, particularly focusing on geriatric populations and those with substance use disorders.

• Interpret and relay critical patient information, including detailed social history, to enhance collaborative care among staff and external providers.

• Coordinate treatment planning, make informed recommendations, and lead decision-making on individual patient care plans, especially focusing on home environments and family dynamics.

• Assess treatment progress and facilitate case review discussions to adapt strategies as necessary for optimal patient outcomes.

• Participate in ongoing professional development and training to maintain high standards of care and compliance with state regulations.

• Complete thorough and timely progress notes post-sessions, ensuring accuracy and clarity to assist in the continuity of care.

• Fulfill additional tasks as assigned to support team operations and enhance service delivery.

Patient Population / Client Focus\n

You will work primarily with a diverse array of patients, encompassing various ages and backgrounds, who present with a range of mental health issues including anxiety, depression, relationship challenges, and substance-related conditions. Understanding the unique needs of each demographic—especially older adults—will be critical in delivering effective and empathetic care.

Qualifications\n

• A Master’s Degree in a relevant field, such as Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling, is required.

• Possession of an independent license is mandatory; applicable licenses include LMHC, LCSW, or LMFT.
