Description American Traveler is seeking a Registered Nurse Case Manager for a Specialty Surgery service line, requiring an active Alaska RN license at time of submittal and at least one year of nursing experience. Details Ambulatory outpatient clinic setting providing patient-centered nursing care across the continuum of care Case management and utilization review responsibilities within a specialty surgery service line Monday through Friday schedule, 0800-1700 Floats to other departments or assignments as needed Requirements Active, unrestricted RN license in Alaska required at time of submittal BLS certification required Case Management certification preferred Minimum of 1 year of RN experience required Graduate of an accredited school of nursing; Bachelor's degree preferred High school diploma or equivalent required Additional Information Performs comprehensive patient assessments and develops individualized plans of care Administers medications and treatments per provider orders and policy Collaborates with providers and interdisciplinary team members to coordinate care, referrals, and follow-up services Documents assessments, interventions, and outcomes accurately and timely in the electronic health record Advocates for patients and helps them navigate the healthcare system Provides culturally responsive, patient-centered care and education Participates in quality and performance improvement activities Assignment start date is contingent on fingerprint card processing, beginning no less than 6 weeks after cards are received Facility does not provide housing or transportation These details are based on information provided by the facility and may include inaccuracies or be subject to updates.
