Key Responsibilities\n

• Conduct thorough assessments of patients to identify mental health needs and tailor treatment plans accordingly.

• Maintain consistent communication with supervisors, direct care staff, and family members to report on individual patient progress and adapt treatment as necessary.

• Provide supervisors with a tentative schedule of patient appointments, home visits, and observation activities to ensure ongoing monitoring of care effectiveness.

• Coordinate home visits to perform assessments, oversee chart reviews, facilitate staff interviews, and analyze collected data for comprehensive case management.
\n
• Ensure that treatment plans are implemented effectively, modifying strategies in response to individual patient needs.
\n
• Participate actively in interdisciplinary team meetings and contribute to collaborative problem-solving efforts regarding patient care strategies.
\n
• Document patient progress through monthly and quarterly reports, ensuring compliance with organizational standards and regulatory requirements.
\n
• Lead training sessions for new staff members to ensure they are equipped with the skills to implement treatment plans and complete necessary documentation accurately.

• Perform follow-up interviews and home visits to provide ongoing support for patients in accordance with their treatment objectives.

• Act as a liaison with community mental health organizations to support development goals and enhance collaborative care opportunities.

Patient Population / Client Focus\n

As a Licensed Clinical Mental Health Counselor, your work will include collaborating with a wide range of populations, including adults and youth facing various mental health challenges. Our patients often require support for issues such as anxiety, depression, trauma, substance misuse, and relationship difficulties. Your expertise in family therapy, couples therapy, or trauma-informed care will be invaluable in meeting the diverse needs of our patient base.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Current, unrestricted licensure as a Professional Clinical Counselor (LPCC) in California is mandatory.
