Key Responsibilities\n

  Conduct comprehensive psychosocial assessments to understand clients' mental health concerns, strengths, and available resources. This includes evaluating emotional and psychological states, family dynamics, and social support systems.

  Provide individualized, family, and group therapy sessions employing evidence-based treatment modalities tailored to the unique needs of clients, including cognitive-behavioral therapy and trauma-focused therapy.

  Develop and implement personalized treatment plans that encompass defined goals and targeted interventions, revising as necessary based on progress and client feedback.

  Collaborate seamlessly with interdisciplinary healthcare teams, including psychiatrists, psychologists, and nursing staff, to ensure holistic and coordinated care for each client.

  Offer timely crisis intervention by assessing risks, formulating safety plans, and providing therapeutic support to clients in distress.

Patient Population / Client Focus\n

In this role, you will primarily support hospice patients and their families navigating the emotional and psychological challenges that arise during end-of-life transitions. You will address an array of complex issues, including feelings of grief, loss, anxiety, and depression, providing compassionate, client-centered care that aligns with their specific needs and circumstances.

Qualifications\n

Compensation and Benefits\n
