Key Responsibilities\n

• Craft individualized care plans by conducting comprehensive assessments of clients’ emotional, psychological, and social needs.

• Actively engage in a multi-disciplinary team approach, collaborating with healthcare professionals—including physicians, nurses, and chaplains—to create a holistic treatment plan that addresses the needs of our hospice clients.

• Monitor and evaluate clients’ emotional well-being through regular follow-up sessions, ensuring they receive adequate support and making necessary adjustments to their treatment plans as required.

• Provide direct counseling services to clients and their families, facilitating discussions that promote healing and acceptance during challenging emotional periods.

• Train and support hospice staff in understanding mental health challenges, fostering a compassionate environment for our clients.

Patient Population / Client Focus\n

The primary focus of this role is to support individuals in hospice care who are experiencing terminal illnesses. You will work with clients facing complex emotional issues, such as grief, loss, and existential distress, providing not only therapeutic support but also guidance for their families. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current LCSW licensure is mandatory to practice in California.
