Key Responsibilities

As a Physical Therapist in our Home Care program, you will have the following responsibilities:

• Conduct thorough evaluations of patients recovering from injuries, surgeries, or chronic health issues within their home environment.
• Develop and implement individualized treatment plans utilizing a range of techniques including therapeutic exercises, manual therapy, and assistive device training.
• Continuously monitor and document patient progress, adjusting treatment strategies as necessary to achieve optimal outcomes.
• Educate patients, families, and caregivers on effective self-care practices and therapeutic exercises, ensuring they feel confident in managing their rehabilitation journey.
• Collaborate with interdisciplinary teams to coordinate care and enhance treatment efficacy, attending team meetings when necessary.
• Stay current with best practices in physical therapy through professional workshops, community networking, and participation in local PT associations.

Patient Population / Client Focus

The patients under your care will range from children to seniors, each with unique rehabilitation needs stemming from various health conditions, injuries, or surgeries. Qualifications

To be considered for this role, candidates must possess:

• A Doctorate degree in Physical Therapy (DPT) from an accredited program.
• A valid state license to practice Physical Therapy in Florida.
• Excellent verbal and written communication skills to effectively interact with patients, families, and healthcare team members.
• Basic proficiency in electronic health records and other clinical documentation systems.
• A flexible and proactive mindset, with the ability to adapt to different patient needs and situations.

Preferred Experience

While prior work experience is not a strict requirement, candidates with at least two years of relevant clinical experience in a physical therapy setting will be given preference.
