Work Environment

As a Physical Therapist at Life Point, you will work primarily in the clients' homes, providing care in a comfortable and familiar setting. Key Responsibilities

• Design and implement individualized physical therapy treatment plans that focus on preventing disability, relieving pain, and maximizing clients' performance capabilities.
• Conduct thorough evaluations of clients' physical abilities and home environments, determining the resources needed to facilitate independence.
• Provide direct physical therapy services, including manual therapeutic exercises, application of physical agents, and instruction in non-manual exercises.
• Collaborate with physical therapy assistants to ensure effective implementation of treatment plans, fulfilling oversight responsibilities as required by state law.
• Prepare and maintain complete clinical records, including evaluations, progress notes, and discharge summaries, while adhering to OASIS standards.
• Participate actively in interdisciplinary team meetings to discuss patient care strategies and modifications, ensuring open communication with all members involved in clients' care.
• Educate clients and their families regarding their physical therapy programs, equipment care, and expected outcomes.
• Stay accessible to assist Physical Therapy Assistants in treatment sessions and be a resource for their questions.
• Contribute to the continuous improvement of care plans based on observations, reporting any changes in the patient's condition to supervising clinicians.

Qualifications

• Current CPR certification and an understanding of federal, state, and governing agency regulations related to healthcare are essential.

Compensation and Benefits
