Key Responsibilities

• Conduct thorough evaluations that encompass historical health information, physical assessments, and the development of individualized treatment plans tailored to each patient's needs.
• Effectively communicate evaluation results and treatment plans to patients and their families, ensuring their active participation in setting achievable rehabilitation goals.
• Implement evidence-based treatment protocols while consistently monitoring the patient's progress and adapting the plan of care as needed based on clinical feedback and patient response.
• Coordinate with interdisciplinary healthcare team members to address patient concerns and modifications, ensuring a holistic approach to rehabilitation.
• Document all patient interactions and therapy services in compliance with regulatory standards, providing accurate and timely information on patient care for ongoing treatment assessments.
• Participate in departmental meetings and contribute to the continuous improvement of clinic operations and patient services.
• Engage in community marketing initiatives to promote access to care and awareness of the services offered by Fairview Rehabilitation Center.

Patient Population / Client Focus

Qualifications

• Bachelor's degree in Physical Therapy is required; a Doctorate in Physical Therapy is preferred.
• Current state physical therapist license for Florida or eligibility to obtain one.
• Certification in Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.

Preferred Experience

Compensation and Benefits
