Work Environment

This is an onsite, full-time position primarily focused on providing home care services to patients within the North Fort Myers region. Key Responsibilities

• Conduct comprehensive patient evaluations by obtaining pertinent medical histories, efficiently performing physical assessments, and establishing tailored treatment plans based on individual patient needs.
• Communicate evaluation findings and treatment strategies clearly with patients and their families, ensuring their involvement in goal-setting and treatment processes.
• Administer effective treatment protocols, continuously adapting approaches based on ongoing evaluations of patient progress and any changes in their health status.
• Consult with fellow healthcare professionals regarding patient care to ensure a holistic approach and timely adjustments to treatment plans as necessary.
• Complete discharge planning that includes follow-up care, while meticulously documenting patient encounters, progress notes, and discharge summaries to maintain high-quality care records.
• Manage time efficiently while meeting productivity benchmarks and maintaining an organized appointment schedule.
• Engage in community and hospital marketing efforts, showcasing the clinic's services and enhancing outreach.

Qualifications

• Doctorate degree in Physical Therapy (DPT) from an accredited institution.

Compensation and Benefits
