Key Responsibilities

• Conduct comprehensive evaluations by reviewing patient histories, performing detailed assessments, and identifying specific rehabilitation needs to develop personalized treatment plans.
• Implement evidence-based treatment protocols that align with patient evaluations and clinical guidelines, ensuring high standards of care are consistently met.
• Document patient progress accurately and timely in accordance with regulatory requirements, providing clear and organized clinical notes and discharge summaries that reflect the patient's treatment journey.
• Communicate effectively with an interdisciplinary team, patients, families, and third-party payors to ensure a continuum of care and manage any changes in patient conditions promptly.
• Attend and actively participate in departmental meetings and training sessions to stay informed about best practices and organizational updates.
• Facilitate discharge planning, ensuring patients have a clear understanding of follow-up care and resources to maintain their rehabilitation progress.

Patient Population / Client Focus

As a Physical Therapist at OnTrack, you will primarily serve orthopedic patients, including those recovering from surgery, injury, or chronic pain conditions. You will be instrumental in developing individualized treatment plans that accommodate diverse needs, empowering patients to achieve their rehabilitative goals through both manual therapy and exercise rehabilitation modalities.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is required.
• Current licensure to practice physical therapy in Florida is mandatory.
• American Heart Association's BLS for Healthcare Providers CPR certification is required.

Preferred Experience

We are seeking candidates with a minimum of 2 years of clinical experience in outpatient orthopedic settings.
