Key Responsibilities

• Conduct thorough evaluations of patients in a home care environment, identifying their specific needs following injury, disease, or surgery.
• Develop comprehensive and individualized treatment plans that incorporate diverse methodologies such as exercise regimens, manual therapy, and the use of specialized equipment to alleviate pain and promote mobility.
• Implement therapeutic interventions and track patient progress, adjusting treatment plans as necessary to meet evolving patient needs.
• Document all treatment sessions, responses, and progress meticulously to ensure continuity of care and facilitate effective communication with other healthcare team members.
• Advise and fulfill the need for assistive devices that aid in self-care and mobility, ensuring that patients have the tools necessary to succeed in their rehabilitation.
• Provide education and training for patients, families, and caregivers on the proper use of modalities and physical therapy techniques, enhancing their understanding of the recovery process.
• Stay updated on industry best practices by attending professional workshops and reviewing relevant literature, thereby ensuring the highest standards of care.
