Key Responsibilities\n

• Conduct comprehensive assessments to determine individual patients’ mental health needs through both virtual and in-person sessions.

• Maintain clear and ongoing communication with supervisors, direct care staff, family members, and other stakeholders to discuss patient progress and needs.
\n
• Craft and distribute a tentative schedule of appointments, home visits, and observations to supervisors to ensure continuity of care.
\n
• Coordinate and conduct in-home visits for evaluations, participant observations, and necessary data analysis that supports our clinical interventions.
\n
• Ensure effective execution of tailored treatment plans while participating actively in interdisciplinary team meetings to discuss strategies and patient metrics.
\n
• Document monthly and quarterly progress notes that reflect the treatment journey and improvements of each patient.
\n
• Lead training sessions for staff to reinforce program implementation and develop new documentation standards for better service delivery.
\n
• Provide follow-up assessments and necessary home visits to continue supporting the evolving needs of individuals throughout their therapeutic journey.
\n
• Maintain organized records, ensuring that all key information is easily accessible to pertinent parties to facilitate informed treatment decisions.

• Serve as a liaison between our organization and the broader mental health community to promote our goals and support clients effectively.

• Contribute to developing training resources and materials aimed at enhancing the competency of staff in program delivery and documentation practices.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Possession of active and unrestricted licensure as a Licensed Professional Counselor (LPC) within the state of Kansas.

• A minimum of two years of counseling experience, ideally in a clinical context.

• A strong foundation in legal and ethical standards of mental health practice.

• Proven ability to establish rapport with a variety of clientele through excellent interpersonal and communication skills.
