This position is designed for counselors who thrive in a remote work environment, providing flexibility to manage their schedule while ensuring that client needs are met. You will be working primarily in an outpatient office setting, offering a blend of telehealth and in-person counseling to patients, which allows for greater accessibility to mental health resources.

Key Responsibilities\n

• Conduct thorough ongoing assessments to identify the unique needs of each patient, tailoring your approach to fit their individual circumstances.

• Maintain open lines of communication between supervisors, families, and other stakeholders to provide updates on patient progress and to coordinate care effectively.
\n
• Develop and communicate a tentative schedule for patient appointments, home visits, and other necessary observations to monitor treatment advancement.
\n
• Organize and participate in essential home visits to facilitate in-home observations and gather critical data to inform patient treatment plans.
\n
• Ensure effective implementation of individualized program plans and adaptations as required by ongoing patient evaluations.
\n
• Contribute actively to Interdisciplinary Team Meetings and other relevant discussions to enhance collaborative care strategies.
\n
• Document monthly and quarterly progress notes, providing insights into the therapeutic journey and adjustments for ongoing care.
\n
• Lead training initiatives to ensure all staff and team members are proficient in program procedures and documentation practices.
\n
• Provide ongoing follow-up through interviews, home visits, and monitoring to ensure continued patient support and success.
\n
• Serve as a liaison between the organization and the local mental health community, fostering relationships that aid in patient development and resource sharing.

• Assist in the development of training materials and procedural documentation to enhance program efficiency and effectiveness.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) within the state of Kansas.

• A minimum of 2 years of counseling experience in a clinical context, demonstrating a commitment to patient care and professional development.
