Key Responsibilities\n

• Conduct detailed assessments and create personalized treatment plans aimed at addressing client needs, which includes developing pre-crisis and crisis management strategies.

• Document all client-related activities in compliance with regulatory standards to ensure accurate record-keeping and care continuity.

• Utilize evidence-based screening tools as part of a collaborative effort to assess individual needs effectively.

• Work alongside clinicians and support staff to implement and monitor positive behavior support plans tailored to each client.

• Engage in all licensing and accreditation processes to uphold the highest standards of care.

• Act as a liaison for clients with relevant agencies, state systems, and community service providers, ensuring they receive appropriate resources and support.

• Develop strong communication channels within the organization and the community, fostering collaborative relationships to meet contractual and operational goals.

• Identify opportunities for partnerships and funding within local, state, and federal agencies to enhance service offerings.

Patient Population / Client Focus\n

Your clients may include individuals dealing with emotional distress, those in recovery from substance abuse, and patients with developmental disabilities or brain injuries. The diversity of our client population requires a counselor who can adapt their therapeutic approach to meet varied needs, emphasizing individualized care and evidence-based interventions to improve patient outcomes.

Qualifications\n

• A Master’s Degree in a behavioral health field is required.

• Possession of an independent LMHC license issued by the state board.

Compensation and Benefits\n

Grace Hospital offers a competitive salary ranging from USD 86,000 to USD 103,000 annually, based on experience and qualifications.
