Work Environment\n

This position allows for a remote work setting, enabling flexibility while maintaining a strong connection with our community health clinic's mission and values. Working Monday to Friday, from 9:00 AM to 5:00 PM, you will participate in virtual consultations, using evidence-based practices through our telehealth framework. Key Responsibilities\n

The LCSW will play a vital role in the mental health team by:

• Conducting and electronically documenting thorough biopsychosocial assessments to address mental health, substance use, and related medical needs among patients.
\n
• Providing high-quality behavioral health services, including but not limited to individual and group counseling, crisis intervention, diagnosis, treatment planning, and referrals for additional care as needed.

• Utilizing advanced clinical knowledge to perform suicide risk assessments, developing safety plans, and escalating critical patient needs to medical providers when necessary.
\n
• Collaborating with Quality Improvement teams to implement and refine data collection processes, enhancing service delivery and patient care outcomes.
\n
• Offering social work support by serving as a liaison to local resources, including community agencies and medication assistance programs, ensuring patients receive comprehensive care.

Patient Population / Client Focus\n

As a social worker at Caring Connections, you will work with a diverse client population, ranging from adolescents to adults, who may be facing various challenges including mental health disorders, substance abuse issues, and socio-economic barriers. Qualifications\n

To be considered for this role, candidates must meet the following qualifications:

• Active and unrestricted License as a Clinical Social Worker (LCSW) in the state of California.

• A Master’s degree in Social Work with a minimum of three years of post-licensure experience in a clinical setting.

• Strong understanding of counseling techniques, crisis intervention strategies, and the ability to make independent clinical judgments.

Compensation and Benefits\n
