Key Responsibilities\n

• Conducting in-depth psychosocial assessments to identify and evaluate clients' mental health needs and available resources.

• Providing evidence-based therapeutic interventions, including individual counseling, family therapy, and group sessions tailored to the unique challenges of hospice clients.

• Collaborating closely with medical professionals and support staff across various disciplines to ensure integrated care for clients, focusing on holistic wellness.

• Serving as an advocate for clients, navigating and facilitating access to community resources and support services tailored to their specific circumstances.

• Engaging actively in case conferences and continuing education workshops to enhance your clinical skills and remain informed about best practices in hospice and palliative care.

Patient Population / Client Focus\n

Your primary focus will be on patients receiving hospice care, alongside their families, who require emotional and social support during a pivotal life transition. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Oregon.

Compensation and Benefits\n

The compensation package for this role ranges from USD 115,000 to 130,000 annually , contingent upon your experience and caseload management.
