Key Responsibilities\n

• Conduct comprehensive assessments to determine clients' needs and design individualized treatment plans.

• Communicate effectively with team members, family members, and caregivers about clients' progress, ensuring everyone involved is informed and aligned on treatment strategies.

• Provide a preliminary schedule of appointments, home visits, and required observations to monitor clients' progress and engage in ongoing assessments.

• Coordinate home visits as necessary to obtain crucial data through chart reviews, interviews, and direct observations of clients' environments.

• Assure efficient implementation of established treatment plans, making modifications as necessary to enhance outcomes.

• Participate in Interdisciplinary Team Meetings to share updates, strategies, and insights pertaining to client care.
\n
• Prepare and maintain meticulous monthly and quarterly progress notes to track client developments.
\n
• Organize training sessions for team members to improve staff competencies in the execution of treatment plans.

• Conduct follow-up engagements through interviews and home visits to ensure continued support and assessment of clients' evolving needs.

• Act as a liaison with the broader mental health community to promote collaborative care and share resources.

Patient Population / Client Focus\n

Our LPC will work with a diverse range of populations, including adults and youth, who may be facing various mental health challenges. You will be instrumental in providing counseling for issues such as anxiety, depression, trauma, and family dynamics, offering therapy that meets the individual needs of each client.

Qualifications\n

• Master's or Doctoral degree in Counseling, Psychology, or a related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in Arizona.
