Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, you will be responsible for a wide range of clinical duties aimed at fostering patient well-being and education:

• Provide clinical behavioral health services tailored to the unique needs of patients, ensuring adherence to the organization’s mission and quality standards.

• Assist mental health consumers in navigating the complexities of both physical and behavioral healthcare systems, promoting integrated health solutions.

• Educate individuals about available resources and services to encourage proactive participation in disease prevention and healthy lifestyle choices.
\n
• Engage in multi-disciplinary treatment team meetings to discuss and optimize patient care strategies.
\n
• Report incidents of crisis or unusual occurrences promptly to the Medical Director, and assess psychiatric and medical crises urgently with the Chief Medical Officer.

• Maintain meticulous documentation within clinical records and other assigned areas, ensuring compliance with organizational standards.
\n
• Participate in the Quality Improvement program, contributing insights and suggestions during assigned meetings and training sessions.
\n
• Collaborate with healthcare teams using evidenced-based screening tools to assess healthcare needs, documenting and reporting results effectively.
\n
• Work closely with patients experiencing Opioid Use Disorder as part of CHDC’s Center of Excellence Team, delivering comprehensive care and support.

Qualifications\n

• A Master’s degree in Social Work is mandatory.

• Licensure as a Clinical Social Worker (LCSW) is required.

• Willingness to travel occasionally to various CHDC locations for meetings, retreats, and outreach events as needed.

Compensation and Benefits\n

We offer a competitive salary ranging from USD 90,000 to 100,000 annually, commensurate with skills and experience.
