Work Environment\n

This role is primarily remote, allowing for flexibility while collaborating with an interdisciplinary team of healthcare professionals, nurses, and caregivers. The work hours are typically Monday through Friday, from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as needed to meet client demands. As part of our commitment to continuous care, on-call availability will also be a requirement. Key Responsibilities\n

• Conduct comprehensive assessments to understand the emotional, mental, and social needs of senior clients.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker at our facility, you will primarily work with elderly individuals facing various psychosocial challenges that accompany aging, including chronic illness and end-of-life issues. Your expertise in geriatric social work will be essential in promoting their mental, emotional, and social well-being, thereby enhancing their overall quality of life.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
