This remote role allows for flexible work arrangements, enabling you to provide critical support to clients without the constraints of a traditional office setting. Despite the remote nature, you will remain closely connected with a network of healthcare professionals, including nurses, physicians, and other social workers, through regular virtual meetings and collaborative care planning sessions.

Key Responsibilities\n

• Develop and implement individualized support plans that address the unique challenges faced by hospice clients and their families.

• Conduct comprehensive interviews and assessments to evaluate clients’ emotional and psychological needs, strengths, and areas for growth.

• Utilize evidence-based practices to create measurable goals, monitor clients' progress, and adjust treatment plans based on ongoing evaluations.

• Provide counseling, resources, and information regarding available community services that can support clients during their journey.
\n
• Document all client interactions and interventions meticulously to ensure continuity of care and compliance with regulatory standards.

• Participate in care conferences to discuss client progress, share insights, and coordinate efforts with the interdisciplinary team.

• Serve as a key worker, advocating for clients and their families by connecting them with external resources and support systems.

• Offer testimony in court or participate in legal proceedings as required, drawing on your expertise and documentation of client care.

Qualifications\n

Compensation and Benefits\n
