Work Environment

This role is performed onsite in a home care setting, allowing Physical Therapists to engage closely with patients in familiar environments. Key Responsibilities

• Deliver comprehensive clinical support to patients, their families, and caregivers, providing guidance on rehabilitation exercises and therapy progression.
• Continuously monitor and assess patient responses to treatment interventions, adjusting care plans as necessary to optimize outcomes.
• Adhere to all organizational and governmental policies, guidelines, and best practices to ensure compliance and improve service delivery.
• Devise individualized treatment plans for residents and patients dealing with temporary or long-term disabilities, focusing on pain relief, functional restoration, and overall healing.
• Ensure that all treatments are conducted in accordance with an established plan of care, documenting progress and updating records as required.

Qualifications

• A current, active license to practice as a Physical Therapist in the state of Florida.
• A Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
