Work Environment

Key Responsibilities

• Conduct comprehensive evaluations of patients to assess their physical therapy needs in the home care environment.
• Design and implement individualized treatment plans that use a variety of methods, such as exercise rehabilitation, manual therapy techniques, and the use of specialized equipment to alleviate pain and enhance mobility.
• Regularly document treatment progress and outcomes to ensure continuous improvement and adherence to established therapeutic goals.
• Reassess and modify treatment plans as necessary to respond to patients' changing conditions and objectives.
• Educate patients, families, and caregivers on effective rehabilitation strategies and the proper use of assistive devices to promote independence and enhance quality of life.
• Stay current with best practices in physical therapy by attending workshops, completing continuing education credits, and networking with other professionals in the field.

Patient Population / Client Focus

Your work will focus on a diverse patient population including children and adults dealing with orthopedic injuries, neurological conditions, post-surgical recovery, and chronic pain management. This role emphasizes delivering tailored therapeutic services that accommodate individual patient needs, ensuring each person receives optimal care during their recovery journey.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) from an accredited institution.
• Possession of a valid State License in Physical Therapy.
• Strong verbal and written communication abilities to effectively convey complex medical information.
• Proficient computer and Microsoft Office skills, particularly in documentation and patient management.
• Flexible approach to working in a dynamic home care environment.

Preferred Experience

• While prior work experience is not required, any background in physical therapy or rehabilitation environments is advantageous.

Compensation and Benefits
