Work Environment

This position is predominantly onsite, allowing you to work directly in patients' homes, which fosters meaningful connections and deeper understanding of their daily challenges. You will collaborate closely with a multidisciplinary team, including occupational therapists, nursing staff, and healthcare aides. Key Responsibilities

• Deliver one-on-one patient care, assessing and designing individualized treatment plans based on thorough evaluations of patients' needs.
• Accurately document all aspects of patient care, including clinical services, billing information, and correspondence, ensuring compliance with established healthcare regulations.
• Utilize your expertise to demonstrate comprehensive clinical knowledge during all patient interactions, ensuring that patients feel heard and supported.
• Maintain adherence to all relevant regulatory requirements, including HIPAA guidelines and Medicare rules, to ensure patient confidentiality and compliance.
• Effectively communicate with patients, their families, and fellow healthcare professionals to ensure seamless coordination of care.

Patient Population / Client Focus

Your primary focus will be on a diverse patient population, including individuals recuperating from surgical procedures, managing chronic conditions, and addressing mobility challenges. Qualifications

• Master's or Doctorate degree in Physical Therapy from an accredited institution.
• Valid Florida Physical Therapy License.

Preferred Experience

• A minimum of one year of clinical experience in an acute care hospital setting.
• Experience in home health care or similar environments is preferred.
