Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to determine clients' needs, concerns, strengths, and resources related to hospice care.

• Provide individualized and group therapy, employing evidence-based practices such as cognitive-behavioral therapy and solution-focused therapy.

• Develop, implement, and monitor customized treatment plans that involve key stakeholders including family members and other healthcare providers.

• Facilitate crisis intervention, complete risk assessments, and develop safety plans when necessary, ensuring the wellbeing of our clients.

• Maintain detailed and accurate documentation that meets ethical and legal standards, contributing to the overall quality of care provided.

• Conduct group therapy sessions, psychoeducation classes, and support groups to empower clients and their families.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

Compensation and Benefits\n
