Work Environment

This full-time position is set within the home care environment, enabling our Physical Therapists to deliver customized treatment directly to patients' residences. Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to assess their physical abilities, limitations, and needs.
• Develop and implement personalized treatment plans, utilizing evidence-based practices that prioritize patient education and active participation in their rehabilitation process.
• Provide skilled therapeutic interventions, including exercises, manual therapy, and modalities, designed to improve function and quality of life.
• Effectively collaborate with a multidisciplinary team, including physicians, occupational therapists, and caregivers, to optimize patient outcomes and ensure a seamless care continuum.
• Educate patients and their caregivers on treatment protocols, home exercises, and strategies to enhance functional independence and prevent re-injury.
• Maintain accurate and timely documentation of patient progress and treatment interventions in compliance with organizational policies and state regulations.
• Achieve and maintain minimum utilization goals and contribute to the organization's overall business objectives.

Patient Population / Client Focus

As a Physical Therapist at Canyon Physical Therapy Center, you will primarily work with patients recovering from orthopedic injuries, surgeries, and chronic conditions. Your patient population may include individuals of varying ages, including the elderly who require specialized care to regain mobility and enhance their quality of life. Your expertise will help patients transition from rehabilitation back to their daily routines with confidence and improved physical functionality.

Qualifications

• Bachelor's or Doctorate degree in Physical Therapy from an APTA or CAPTE accredited program.
• Current State License in Physical Therapy valid in Florida.
• Basic Life Support certification from the American Heart Association.
• Excellent written and verbal communication skills to effectively convey complex information to patients and families.
• Basic proficiency in word processing and digital documentation platforms is required.
• Strong interpersonal skills that foster meaningful relationships with patients and their families, ensuring adherence to treatment plans and engagement in recovery.

Preferred Experience

• Prior experience in a home care setting is highly preferred, showcasing your adaptability to providing care in various environments.

Compensation and Benefits
